WorkSafe is investigating the circumstances surrounding the death of an 84 year-old woman following an incident at a Horsham hospital on Monday afternoon.

While investigations are continuing, WorkSafe understands the woman was lying on a mobile showering trolley when one of the side guard rails dropped, resulting in the woman falling to the floor and sustaining head injuries. She died that evening.

WorkSafe visited the hospital yesterday to commence an investigation into the cause of the incident. As part of the investigation, WorkSafe has issued an improvement notice requiring the trolley be tested.
